Narga Medulla is a material that can be obtained in High Rank quests as well as during Rampage Quest by taking down Nargacuga. In Rampage quests, you can obtain it as a drop or as a reward for completing the quest, but it is not the fastest way of getting it, considering how long Rampage quest can take to complete, and all the other monsters you have to face during them.

The absolute best way to get Narga Medulla is by taking down a Nargacuga in High Rank quests. You have a 12% chance of getting it by carving the monster’s tail, a 7% chance of getting it by carving the monster after killing it, a 5% chance of getting it as a target reward and a measly 3% chance of getting it by capturing the monster. As such, you should focus on cutting the monster’s tail, which shouldn’t be too difficult to do if you focus on this task, given how big it is.
