How to Get Eye of the Witness in Cult Of The Lamb
To get the Eye of Witness in Cult of The Lamb, you must clear the Crusades in Darkwood, Anura, Anchordeep, and Silk Cradle and then re-enter them to find new mini-bosses, aka Witness, and then defeat them to obtain a chest that contains the Eye of the Witness.

For the first time, when you enter Darkwood, you will fight Leshy, the main boss, often called Bishops. Similarly, you encounter Kallamar in Anchordeep, Shamura in Silk Cradle, and Heket in Anura. You will find all these bosses at the near end of each Crusade. Defeat them.
Once that is done, you can find the NPC named Plimbo at the Smuggler’s Sanctuary to start the Eye of The Witness quest that you can complete if you have four Eye of The Witness.
As mentioned, re-do the Crusades to defeat the new mini-bosses to spawn the chest with the Eye of The Witness inside them. When you re-enter the Crusade, you will get a warning from an NPC named Haro that the enemies are now much stronger and all that.

But who cares? Right? You will also be much stronger at that point. Well! No! Be cautious while re-doing the said Crusades, as the new mini-bosses are just termed “mini” but are somewhat challenging. So, take on the battle only when you are fully prepared.
